usatoday.com A TransAsia Airways passenger plane crashed Wednesday into residential buildings in an emergency landing in Taiwan's Pengu Islands, killing scores of people, according to news reports. The Taiwan news agency CNA initially put the death toll at 51 and later reported as many as 45 dead and 9 injured.

AP A plane making a second landing attempt in stormy weather crashed at an airport on a small Taiwanese island late Wednesday, killing 51 people and injuring seven, fire officials said. Penghu is a lightly populated island that averages about two flights a day from Taipei.

reuters Pro-Russian rebels have shot down two Ukrainian fighter jets, a spokesman for Ukraine's military operations said. The spokesman said the two Sukhoi-25 fighters were downed around 1.30pm local time near Savur Mogila in eastern Ukraine.

Once I discovered site airdisaster.com , as a frequent flyer, I read almost all stories and transcripts, so the main reasons of disasters were at first place bad weather conditions (often coupled with airports by a hill), then human error them technical malfunctions.

So upon such discovery I limit flying during a night or late afternoon to possibly poorly equipped regional airports, and postpone it during bad weather.

As well now I better understand why in PhangNga West coast there is no airport - mountains as it cited officially - which is a sufficient reason itself, and then on top of that green season weather.

The main cause of almost all aircraft crashes is human error. Aircraft crashes are rarely caused by one factor or failure, they result from a number of contributing factors then by analysis of these the root cause is determined.
Although the human factor may not be the root cause, it contributes to almost every accident.
Even in MH17, human error will eventually be a contributing factor whether that be with the airline or the aviation authority having jurisdiction in the area or by the people who fired the missile or by all to varying degrees.
